➤ Step by Step instructions:
1. Go to the “Admin Settings” menu.
2. Please select “Questionnaires”.
3. Click on Create new to start the process.
4. Select one of the available languages, such as Greek or English.
📌Note: Make sure you adapt the content in all languages, so it is visible to all user profiles.
5. Click on the Create new (with the pencil icon), to start manual entry.
6. Fill in the following basic fields, if required:
- Questionnaire title (mandatory field): The title is the primary name of the questionnaire. It is used by the administrator to identify and properly organize questionnaires in the system.
- Questionnaire header (optional field): The header is for internal use only and is not displayed to the supplier. Ιt can be used to enter additional information that helps the administrator.
- Questionnaire footer (optional field): The footer can be used for additional notes or comments intended exclusively for the administrator and is not displayed to the supplier.
- Valid for months (optional field): Τhis field defines the questionnaire’s validity period (in months) and is useful when a specific timeframe needs to be set. It is for informational purposes only and does not affect the questionnaire’s functionality.
- Vendor prompt (mandatory field): This field represents the questionnaire title as displayed to the supplier. If the field remains blank, the title will not appear on the supplier’s view.
7. Select the “Approval after submission” checkbox if you want the option to evaluate the Questionnaire per response or as a whole.
📌Note: If this option is not selected, the questionnaire will be approved automatically. For more information, see the article:

8. Click the +Add question button to start entering your questions.
9. Enter your first question in the corresponding field.
10. Select how you want to receive the supplier’s answer by choosing the appropriate Question type. To view the available question types, see the article:

11. By selecting the “Needs approval” checkbox (within the question you are creating), you also have the option to approve or reject that specific question.
📌Note: If your questions are part of a questionnaire that you want to be approved automatically, you should not select the “Needs approval” checkbox. See details about the questionnaire evaluation in the following article:

12. Add as many additional questions as you want by clicking the +Add question button each time.
13. Click the Save Questionnaire button to save the questionnaire.
